Interim care

Interim care is for clients who are discharged from hospital and require a transitional period of residential care support services for a short period of time After this period, the client will either return to hospital or be discharged home.

For clients who are identified as needing long term or permanent residential care placement following discharge, refer to long term care.

Referral for support services on discharge

Sometimes when a client is discharged from hospital they are unable to return to their own home. This may be due to environmental or access issues and/or need for more rehabilitation before they can safely return home. Hospital staff can refer the patient to support services, including interim residential care, by completing an ACC705 form. The client may then receive rehabilitation while in interim residential care to facilitate their return home.

Extension to care or change in care requirements

Over time, a client’s needs or required level of care may change.

In such cases, the Residential Facility must complete an ACC1156 Variation Report - Residential Support  to notify us of these changes. This form is used to request funding for a higher or lower level of care or to request an extension in support.

ACC will review the request, confirm if the change or extension can be approved, or request further information if needed.
